Writing one
Write the plan against a section and subject for the period it covers. A draft is yours until you submit it.
Review
Submitting sends it to your head of department. They can approve it, or return it with comments for you to revise and resubmit.
The whole exchange is kept — who submitted, who reviewed, when, and what was said. That trail is the point of the workflow, not a side effect of it: it is what lets a school show its lesson planning was actually supervised.

If it is not reviewed
The plan sits with the reviewer. Chase the person, not the screen — nothing here escalates on its own.
